  • Click on Book Now button at the top of the page. 
  • Read the important notice and undertaking carefully.
  •  Fill the complete application form by providing the necessary details.
  • Send the appointment fee on any of the given payment methods on the website.
  • Attach a screenshot of the payment before submitting the form.
  • Please read and accept the terms and conditions by selecting the checkbox. Submit the application form by clicking the submit button.
  • Once you have paid the fee and submitted the form, you will receive your GAMCA medical slip within one hour.
